Fairview Home Infusion

Fairview Home Infusion (FHI), a division of Fairview Pharmacy Services, is located in Minneapolis, MN. Since 1989 FHI has provided home infusion services to pediatric and adult patients. Currently we are able to support Minnesota, Wisconsin, Iowa, South and North Dakotas, Florida, Arizona, Indiana, Colorado, Nebraska, Washington, and Idaho. We provide Biologics, Immune globulin (IV and subcutaneous), TPN, Antibiotic/Antimicrobial therapies, Chemotherapy, Inotropes, Pain Management, and Enteral. Penn Home Infusion Therapy, certified by the Joint Commission, provides patients with necessary treatment and intravenous medication in the comfort of their own home, which can lead to more flexibility and independence in their daily routines. Our infusion services are available in Pennsylvania, southern New Jersey, and northern Delaware, and our multidisciplinary team approach allows us to provide continuity of care from hospital to home.

Allegheny Health Network Specialty Home Infusion provides care for patients in Western Pennsylvania. Through the Allegheny Health Network Home Infusion program, patients can receive intravenous medications and nutritional support in the privacy and comfort of their own homes. AHN’s Specialty Home Infusion program offers therapies for a wide-range of complex conditions including Crohn’s disease, ulcerative colitis, amyotrophic lateral sclerosis, immune deficiency disorders, psoriasis and psoriatic arthritis, rheumatoid arthritis, myasthenia gravis, and multiple sclerosis. AHN Home Infusion also offers at-home therapies including antimicrobials, parenteral nutrition, chemotherapy, ionotropes, pain management, and steroids. Their team of pharmacists, nursing infusion specialists, nutrition support teams, and pain management and supportive care teams provide support for their patients.

UW Health Care Direct (UWHCD) is a Joint Commission accredited home infusion provider located in Middleton, Wisconsin serving patients in Wisconsin as well as some areas of Illinois, Iowa, and Minnesota. UWHCD is a wholly owned affiliate of UW Health and supports two hospital systems: 1) UW Health, which is the integrated health system of the University of Wisconsin-Madison caring for more than 700,000 patients each year, and 2) UnityPoint Health-Meriter, which is a partner of UW Health and highly respected community hospital.
